Home Health Care
Home health care allows individuals who are recovering from a surgery, injury or illness, managing a serious or chronic disease or dealing with multiple diagnoses, to remain safely at home and live as independently as possible. At Kreston Alphacare Group, our goal is to give you or your loved ones the tools and resources to help control your disease, achieve your health goals and live life to the fullest based on your doctor’s orders and clinical needs. Wherever you or your loved one call home, our team of skilled home health nurses, home health aides, and physical, occupational or other thera- pists, will make regular visits to provide health care home-based services.
Recovery Care
Kreston Alphacare Recovery Care at home, helps health system partners bring acute care to the comfort of patients’ homes. To do so, we build a care team that is fully equipped to follow patients through the continuum of their care, from their initial hospital visit to their admission to the Recovery Care at Home program and throughout their episode. • Recovery Care at Home Points of Entry: Straight f rom the Emergency Department (ED) Recovery Care Coordinators identify eligi- ble patients who would otherwise be admitted to the inpatient unit from the ED. If the admitting physician agrees they can be cared for at home, these patients are offered this option, with more than 90 % choosing to accept care at home.
Rehabilitation Care at Home
With Kreston Rehabilitation Care at Home, patients, providers and health systems experi- ence less fragmentation, better coordinated care and improved treatment progression and outcomes. In addition, over the 60-day model, our patients enjoy sustained engage- ment with their care teams with whom they become familiar. Now, patients and their families can truly experience a sustainable recovery in the comfort and familiarity of their own homes.
Palliative Care Services
Unlike hospice care, palliative care is not associated with anyone specific symptom or health issue. Hospice Care Services
If you or a loved one is facing a terminal condition, hospice may be the answer. Hospice care is a unique, holistic benefit that can help people have a dignified death at home, surrounded by those they love. Hospice is defined as supportive care that focuses on comfort and quality of life for patients with a terminal illness and a limited life expectancy if their disease runs its normal course, as certified by a physician. An interdisciplinary team of compassionate clinicians work together to treat patients’ physical, emotional and spiritual well-being as they near the end of life.
